In order to provide efficient claims processing, we require that you bring your insurance card or cards to all of your appointments.


You are responsible for any charges not covered or balances that remain after processing by your insurance plan.


Checks, cash, credit cards including Visa, MasterCard, and Discover, debit cards, flex cards, and CareCredit are accepted as forms of payment.

Dr. Honl accepts most insurance plans. You can contact our office to see if your plan can be processed. Some insurance plans require you to designate a network.


Services received within your chosen network will be paid at a higher benefit level than if you were to receive services from an out-of-network provider. Dermatology Associates PC is affiliated with the Heartland Independent Providers Network (HIPN).


Not all insurance plans require a network. For specific questions about your insurance plan, please contact your insurance company.

For your convenience, you can print and complete these forms prior to your appointment to save time you would have to spend on registration on the day of your visit.

Remember to fill out our forms if you are a new patient or a patient who has not been seen for several years.

Patient accounts with a credit balance will be reviewed on a quarterly basis in January, April, July, and October. Refunds will then be issued to the appropriate party, either the guarantor or insurance company.


A refund may be sent outside of the above dates if identified and requested by the patient or insurance company. Justification must be provided as to why a refund is being requested outside the regular quarterly refund dates.


Refunds will not be issued if the patient has an upcoming appointment within the next 30 days and it is expected that the patient will owe a co-pay, coinsurance or deductible amount.


All refunds will be sent via check to the patient / guarantor at the address on file.
